Once you register your child (fill out the one page form with your child at: http://www.igamelab.com/default.php?page=join.php) please fill out the form below and send to us either by postal mail or facsimile (fax)at the mailing address or fax number provided below . Alternatively, you may copy and paste and fill out the form and email it to: projectmanagement@igamelab.com AND CALL: Projec t Director Aaron at:860-349-0451 and confirm your permission (you may leave a message if he is unavailable). Your child's membership will not be activated until we receive written permission from you or the email and phone call.

I represent and warrant that I am the parent or legal guardian of the child named above, am at least 18 years of age, and possess the legal right to enter into this agreement. I am aware that the purpose of iGameLab.com is to gather opinions from kids aged 9 to 18 about products and services of many kinds-primarily games and websites. I understand that the opinions of panel members gathered in online surveys will be shared with iGameLab.com's clients in aggregate, without identifying the identity of any child, parent or legal guardian. I understand that often, studies will involve a parent bringing a child to a "usability lab"-a special facility in the Los Angeles area where he/she will play a game or check out a website/website feature and participate in a one-on-one discussion, or small group discussion with other young people and a professional youth moderator. For these in-person studies payment would typically be a approximately $60.00 in cash. Since the sessions at the "usability lab" are in-person and payment is in cash at the completion of the session, participants' identification is known. There is never any obligation to participate in any in-person study, I the parent, choose whether to allow my child to participate. I understand that no child may participate in an in-person study without the presence of a parent or legal guardian. The opinions of panel members will be used to help iGameLab.com learn what kids think about many issues in order to advise companies on the design of games, websites and other products, or services.

By signing this agreement, I confirm that I am aware that all study data collected by iGameLab.com is the property of iGameLab.com/Touchstone Research, Inc./ChildResearch.com and/or its clients, and that iGameLab.com panel members and their parents and legal guardians, agree to make no claim of ownership with respect to such study data collected in iGameLab.com in-person and online activities and any other related research projects/studies.
